1. Simply select the option above that applies to you, and the specific form you require will open in Adobe Reader
2. Fill out the form completely.
3. Print the completed form and sign it.
4. Residential Applications for Service require two pieces of photocopied identification (one with a photo). Preferred ID: Driver’s Licence and SIN.
5. Send us your applications (and photocopies of ID, as required) by fax or mail.
